Filling out these forms ahead of time will help make your visits to Hillside Dental Care as quick and convenient as possible. If you have any questions when filling out the form our staff will happily assist you with any questions on the day of your appointment.
Financial information
All of our patients at Hillside Dental Care are entitled to a free consultation where treatment options, fees and general questions can be discussed. A dental treatment is an important decision so we want to make sure you understand exactly what is being done, the benefits, and the risks. Prior to proceeding with any treatment, fees and financial arrangements will be discussed with you and all your questions will be answered.
Hillside Dental Care accepts cash, personal cheque, debit, Visa, Master Card and Amex. We electronically file insurance claims. As an assignment office we submit your claims electronically to your insurance company and payment comes directly to us.
